I give permission for my child, ___________________________________________

to attend the Big Brothers Big Sisters Mardi Gras party on Saturday, March 29 from

1:00 to 4:00 p.m. at the Franklin Pierce “Bubble”.

I understand that I am responsible for providing transportation to and from the event

for my child.  I also understand that since my child participates in a site-based match,

he/she is not covered by the Big Brothers Big Sisters accident insurance policy for

this event. I will not hold Big Brothers Big Sisters of the Monadnock Region

responsible for any accident or injury to my child resulting from participation in this

event.
